Care Coordinator   

Location: Nottingham

Hourly rate: TBD

Duties and Responsibilities 

• Ensure that the Quality of Service meets with CQC guidelines and expectations.
• Care Coordinator ensures that Regulatory Requirements are met.
• To implement a high-quality service through effective matching of service user needs with suitably qualified and experienced care workers.
• Care Coordinator liaise with the Registered Manager to ensure that there is enough cover to deliver all visits and that new clients are planned into the rota in a timely fashion.
• Ensure working patterns are efficient and planned logically.
• To effectively prioritise tasks to meet unexpected emergencies and changes in care packages.
• Ability to maintain clear and accurate written records and follow statutory reporting procedures.
• Should have ability to communicate clearly and build positive working relationships with service users, their families, fellow staff and other health and social care professionals.
• Receive and handle complaints about services from service users or others.
• Carry out on-call coordinating duties outside normal office hours as agreed.
• Carry out risk assessment, write and review Care Plans.
• Supervise carers.

 Care Assistant

Location: Nottingham

Hourly rate: £11.44ph - £12ph + 35p mileage

• Medication Support: Ensuring clients take their medications as prescribed.
• Practical Tasks: From shopping to mealtimes and housework, you’ll be there to lend a helping hand.
• Documentation: Keeping detailed records of tasks completed helps maintain quality care.
